Annual physical examination includes review of your immunization history and update immunization as needed, general physical exam/lab work and age appropriate cancer screening. 

In order to allow enough time for thoroughly evaluating your general health, please schedule a different office visit for other medical concerns than annual physical exam.

Lab result followup visit includes explanation for lab results, treatment for medication problem(s) indicated by the results and health education. Lab result followup visit after physical exam is processed by insurance company as a separate office visit from annual physical exam.

Medical office is responsible for taking care of patients medically and submitting office visit claim to insurance company. Insurance company processes the claim and decides how much they would pay and how much patient needs to pay. In rare cases, insurance company processes the claim incorrectly or disagrees with the code office provided, office will try the best to cooperate with insurance company to help get the claim covered. However, medical office is not the one who processes the claim. Please communicate with your insurance company, If you have any questions about the claim. 

For high deductible health plan, currently we are not accepting high deductible health plan without existing patient's referral or if you have read and agreed with the entire frequent asked questions. Office visit will be charged at approximate rate upfront without exception. You will be reimbursed or charged for the difference after the insurance company processes the claim. Receipt will be provided upon request after claim finalized.

No, please don't negotiate office visit fee with us. We charge based on insurance guidelines and can not give discount. The fee is the same if you have HDHP or regular PPO. The only difference is you pay or insurance pays.

If you book a vaccine visit, the most important is to get vaccine administrated. You can ask doctor during the visit if you have other issues. Doctor will decide if there is enough time to treat you at the same visit or schedule another visit. But the treatment/consultation will be billed separately and not part of the vaccine visit.

In a concierge medicine practice, patient pays membership fee. Doctor makes a commitment to limit the number of patients to ensure adequate time and availability for each patient. In our practice, you will have 24/7 direct access to doctor, unhasty visit, inpatient care in case you need and discounted rate for anti-aging service etc.
