To ensure enough time for a comprehensive preventive evaluation, additional medical concerns or new symptoms may require a separate office visit.

Our medical office is responsible for providing medical care. Billing and claim submission are processed through El Camino Health. Your insurance company reviews and processes the claim, determines coverage, and decides how much the insurance company will pay and how much the patient is responsible for.

In rare situations, the insurance company may process a claim incorrectly or disagree with the coding submitted. In those cases, our office will do our best to cooperate and assist when appropriate. However, the medical office is not responsible for processing insurance claims or making final coverage determinations. If you have questions regarding your claim, benefits, deductible, or patient responsibility, please contact your insurance company directly.

 Patients with High Deductible Health Plans are generally responsible for medical costs until their deductible is met. Estimated visit charges may be collected at the time of service based on insurance guidelines. After your insurance company processes the claim, any balance adjustment, additional payment, or reimbursement will be finalized accordingly.

Receipts and claim documentation are available upon request.

 Our office follows contracted insurance pricing and standard billing guidelines. Visit charges are based on the services provided and are generally the same regardless of whether you have a regular PPO plan or a high deductible plan. The primary difference is whether the payment responsibility is assigned to the insurance company or to the patient.

 The primary purpose of a vaccine visit is vaccine administration. If additional medical concerns are discussed during the visit, the physician will determine whether there is adequate time to address them during the same appointment or if a separate visit is recommended.

Additional evaluation or treatment may be billed separately from the vaccine administration visit.
